Students expecting to be appointed as teaching assistants, and not previously on the Rutgers payroll through the Department of Statistics and Biostatistics, must provide the following forms. The student is responsible for updating these forms as their personal information changes:

• Biographical data form
• Address Directory Record
• Payroll Forms for students with F1 or J1 visas (only for students on F1 and J1 visas).
• I-9 Form, verifying employee eligibility
• Federal Withholding Allowance Form
• New Jersey Withholding Allowance Form
